How do I get referred to the Sleep Center?

You need to discuss any sleep problems you have with your physician.

Your physician will evaluate whether you need a consultation, a sleep study or both.

Your physician's staff will request authorization from the insurance company or CMG's managed care department and send a request online to the Sleep Center or fax a request. The authorization process should take less than a week.

The staff at the Sleep Center should contact you to set up an appointment. If you have not heard from the Sleep Center in one week, please call 408-523-3460 to schedule your appointment.
